Four Seasons Hospice & Palliative Care has a contractual relationship with both Pardee and Park Ridge Hospitals. When a loved one in the hospital is facing a life-limiting illness, hospice services can be initiated before they leave.
And when your family member is discharged from the hospital, our staff will assist you in the transition to the new location and continue to provide services.
Four Seasons Hospice & Palliative Care employs physicians, nurse practitioners, registered nurses, social workers, chaplains, certified nursing assistants, volunteers and bereavement counselors to deliver an integrated, holistic approach to healthcare, no matter where that person may reside.
